Almost every visit starts the same way. Someone takes a blood pressure, a pulse, a weight, maybe a temperature, and those numbers go into the chart. They are the most routine data a clinic collects, and for a lot of care they are the data that matters most. A blood pressure drifting up over a year. A weight coming down on a new plan. A pulse that settled once a medication took hold. The single reading tells you where a patient is today. The line tells you whether what you are doing is working.

The trouble is the shape the numbers land in. In most charts, vitals get typed into the prose of a note: "BP 138/86, HR 78, wt 84 kg." Readable to you, invisible to the software. To see whether the pressure is climbing you open three old notes and read them side by side, because there is no single field the system can line up for you.

Vitals are easy to record and hard to trend

This is exactly backward from what the data is good for. Blood pressure, weight, BMI, and temperature are numbers, the most chartable thing in medicine, and yet when they live inside sentences they cannot be charted at all. Each visit you write them down again, and each time you want the trend you reconstruct it by hand.

It matters most where care runs long. Hypertension you are titrating across a year. A weight-management plan where the curve is the whole point. A thyroid or cardiac picture where pulse and pressure are the signals you watch. None of those questions can be answered by one reading. They need several, in order, drawn as a line, and a note full of free text will not give you that no matter how carefully you wrote it.

Why is it hard to track a patient's vital signs over time in most EHRs?

In many EHRs, vital signs are typed into the free text of a note rather than stored as discrete, dated values. The software can display the note but cannot read the numbers inside it, so comparing blood pressure or weight across visits means opening several notes and reading them side by side. Because that work is manual, vitals are recorded every visit but their trends often go unseen.

What the Vitals block captures

The Vitals block lives inside a progress note. You do not leave the chart or open a separate screen. Type /vital and pick Vitals from the Common group.

What you get is a set of fields, not a sentence. Blood pressure splits into systolic and diastolic in mmHg, with heart rate in bpm alongside them. Then the anthropometrics: weight, height, and temperature, each with a unit toggle. Weight takes kg or lb, height takes cm or ft and in, temperature takes Celsius or Fahrenheit. You enter whatever the cuff or the scale shows, and Oli stores it in metric underneath so the history stays consistent no matter which unit the room happened to use. A notes field and a date and time stamp round it out, so each set of vitals is pinned to the moment you recorded it.

Every field is optional, so a quick blood pressure check is just two numbers and a pulse, and a full physical fills the rest. The one value you never type is BMI. Enter a weight and a height and Oli computes the BMI from them and keeps it current, so the index always matches the measurements it came from instead of being a number someone calculated once and forgot to update.

BMI is computed, not typed. When you enter a weight and a height in the Vitals block, Oli derives the body mass index from them automatically and stores it alongside the measurements. Because it is calculated from the weight and height on file rather than entered by hand, the BMI always matches the values it came from and updates whenever either one changes.

Recording the visit fills it in

Most of the entry can be gone before you type anything. When the Vitals block is part of your note template and you record the encounter, Oli's AI charting reads the transcript and fills the block as it drafts the rest of the note. If the numbers were said out loud during the visit, the blood pressure, pulse, weight, and temperature land in the right fields instead of sitting in a wall of prose you have to unpack afterward.
